Any student who is not a U.S. Citizen or a U.S. Permanent Resident is required to fill out Page one (Biographical and Immigration Information) of the Financial Certificate Form.

The Financial Certificate is used partly to collect current immigration status for data entry purposes. Students who are not requesting a Forms I-20 for F-1 student status or DS-2019 for J-1 student status are required to complete page one only. Students in an immigration status other than F-1 or J-1 should indicate their current immigration status on the form and attach a copy of their current immigration documents. This includes students who have pending adjustment of status applications for permanent residency with United States Citizenship and Immigration Services (USCIS).

Information provided on page one of the Financial Certificate form assists International Student and Scholar Services (ISSS) in keeping accurate records on our student population and assists us in advising students about immigration options for study at UNC-CH. All non-immigrant students are required to check in with ISSS.
